Mechanicsburg Area School District will again be offering FREE sports physical screening exams for all 7th grade through incoming 12th grade students (2025-26 school year). Please review and follow the instructions below.

• All athletic physical forms are available via the Parent Portal of Sapphire.

• ​School sponsored Pre-Participation Athletic Physicals are for students in grades 7 - 12 who are trying out for an MASD school sponsored sport.

•  Please complete the online forms on the Sapphire Portal. Print the Health History and Pre-Participation Physical Exam form and have your student-athlete bring it with them to their appointment. Forms completed during the school sponsored exam will be returned to the student-athlete for the parent/guardian to upload on the Sapphire Portal. Please do not hit the "submit" button in Sapphire, until you upload the Health History and Pre-Participation Physical Exam form.

• ​All physicals for the 2025-26 school year MUST be dated May 1, 2025 or after.​​
